“Esomeprazole and esomeprazole hydrochloride are both medications used to treat GERD and are the first-line therapies for GERD and ulcers,” said Dr. Lisa M. Diggle, PharmD, of the Gastroenterology Unit at New York University School of Medicine in New York, New York, NY.
“However, they have a different mechanism of action. They work by reducing acid secretion, which can help reduce the risk of ulcers. They may also have a different effect on gastric pH, which may help in reducing the risk of ulcers. Esomeprazole may help reduce the risk of esophageal inflammation. It also reduces the risk of stomach ulcers, which is associated with heart disease.”

Proton pump inhibitors are a popular class of drugs for heartburn, but there’s growing concern about their safety. In a recent study of more than 100 patients, the researchers reviewed more than 1,000 clinical studies that compared esomeprazole, the active ingredient in Nexium, to esomeprazole-based drugs that are used to treat gastroesophageal reflux disease. The findings suggested that patients taking Nexium had a higher risk of developing heartburn, but that the medication’s side effect profile did not appear to be related to long-term use.
“The safety profile of esomeprazole is similar to that of some other drugs,” says lead author Joshua M. Smith, MD, PhD, a gastroenterologist and the lead author of the study.
Esomeprazole was prescribed for more than 50,000 patients in clinical trials, but there is little evidence to show that it causes heartburn. In the study, the researchers used a randomization approach to determine the risk for heartburn and found no significant differences between the esomeprazole and esomeprazole-based drugs when they were compared in separate studies.
“There are no substantial differences in heartburn risk between esomeprazole and other drugs that are used to treat gastroesophageal reflux disease,” Smith says.
Esomeprazole is also prescribed for a broader range of conditions, such as those that affect the stomach and intestines, and it’s commonly used as an adjunctive treatment for chronic ulcers and other conditions related to the stomach.
Smith notes that patients taking Nexium have a higher risk of experiencing a heartburn than patients taking other drugs that are used to treat the condition. For example, patients taking Nexium for ulcerative colitis had a greater risk of a heartburn when they took the drug compared with those taking a placebo.
“While heartburn is a common side effect of esomeprazole, the fact that it was associated with an increased risk of heartburn raises the question of whether it was due to a broader safety profile,” Smith says. “We think that the safety profile is not very specific to esomeprazole and that may be the reason it is so associated with the other drugs that are used to treat the condition.”
Esomeprazole was initially approved by the FDA in 1981 for the treatment of heartburn, and the Food and Drug Administration in 1989 for the treatment of ulcers caused by acid reflux disease. Since then, it has been approved for other conditions such as gastroesophageal reflux disease (GERD), and in patients with chronic reflux disease. Studies of esomeprazole have been ongoing.
Esomeprazole was approved by the FDA in 1999 for the treatment of reflux esophagitis and as a treatment for heartburn, but it has been prescribed for a broader range of conditions, such as heartburn and GERD, and has been used to treat a range of other diseases.
“This study shows that the risk of heartburn in patients taking Nexium is higher than in patients taking other drugs,” Smith says. “So it is important that patients taking Nexium are evaluated for their heartburn risk.”
In addition to the findings, Smith and his colleagues found that patients on esomeprazole had a higher risk of developing heartburn, although these findings were inconsistent in a large, placebo-controlled, multicenter study.
“It is important that patients taking Nexium are evaluated for their heartburn risk,” Smith says. “For this study, we used a randomized, placebo-controlled trial to evaluate the risk of heartburn among patients taking esomeprazole compared with those taking other drugs, and it was statistically significant,” Smith says. “So these patients should be screened for heartburn risk, and those taking esomeprazole should be evaluated for their heartburn risk.
